Registration

Registration is simple, please complete the medical history form below and email to sundondental.practice@nhs.net, you will be placed on our New Patient Waiting List and contacted when we are able to see you.

The practice is currently only accepting children as new patients, under the NHS.

Arrival

Upon arrival at the practice you will be asked a series of questions including your medical history, this allows us to obtain a comprehensive history from you and allows us to treat you in the best and safest possible  way. If you are receiving medical treatment and taking any prescribed medicines please bring a list of these along with you.

In the surgery

The dentist will carry out a thorough examination of your teeth, gums and soft tissues.  They will also assess your occlusion-the way your teeth bite together and address any concerns that you may have. The dentist will ask you questions during this visit and keep you fully informed of any treatment that may be required.  All surgeries are fitted with x ray equipment to help with diagnosis and explanation of any proposed treatment. Written treatment plans and costs will be provided.
